Are you a European-trained doctor considering a move to the UK to work as a consultant or in a senior specialist role? To do so, you'll need to be on the Specialist Register with the General Medical Council (GMC) — a legal requirement for anyone practicing as a substantive consultant in the NHS.
This blog will guide you through everything you need to know, from eligibility and documents to costs, timelines, and the benefits of joining the Specialist Register.
📌 What is the Specialist Register?
The Specialist Register is maintained by the GMC and lists doctors who are eligible to work as consultants in the UK. Being on this register confirms that you have completed specialist training and are fully qualified in your chosen specialty.
✅ Who Needs to Be on the Specialist Register?
Doctors applying for substantive consultant positions in the NHS
Specialists looking to work independently in the UK
European Economic Area (EEA) doctors whose qualifications are not automatically recognised
🛤️ Routes to Join the Specialist Register (for European Doctors)
There are several pathways to gain specialist registration depending on your qualifications and where you trained:
1. Automatic Recognition (via the EU Recognition Route)
Eligibility:
You must hold a specialist qualification awarded in an EU/EEA country that is recognised by the GMC as equivalent to UK CCT standards.
This applies only if your training started before the end of the Brexit transition period (31 Dec 2020), or if you fall under the EU-UK Trade and Cooperation Agreement (TCA) provisions.
Steps:
Create a GMC online account
Complete the online application form for Specialist Registration via the EU route
Submit evidence of training and qualification
Pay the application fee
Wait for assessment and registration
2. Certificate of Eligibility for Specialist Registration (CESR)
If your European qualification is not automatically recognised, you can apply via the CESR route.
Steps:
Check Eligibility
Ensure you can demonstrate skills, knowledge, and experience equivalent to a UK-trained consultant in your specialty.
Gather Evidence
This can include:
Specialist qualification(s)
Curriculum vitae
Logbooks of procedures
Audits, appraisals, assessments
CPD (Continuing Professional Development) evidence
Supervisor reports
Clinical attachments or fellowships
Translations of documents (if not in English)
Create a GMC Online Account
Start an application through the GMC online portal.
Submit Your CESR Application
Upload all documents and evidence of your clinical experience.
Pay the Application Fee
As of 2024: CESR application fee: £1,676
Assessment
The GMC will send your application to the relevant Royal College for evaluation. They compare your experience to the UK CCT curriculum.
Outcome
You may:
Be added to the Specialist Register
Be asked for further evidence
Be advised to gain additional experience
⏳ How Long Does the Process Take?
EU Route: Around 3–6 months
CESR Route: Typically 6–12 months depending on evidence complexity and completeness
